Use this form to submit a formal request to reschedule or cancel your scheduled service.
Please be advised that all requests are subject to our cancellation policy, which requires a minimum of 48 business hours' notice to avoid a cancellation fee. Submissions received after business hours or on weekends will be processed the next business day.
Our scheduling team will review your submission and send a confirmation once the change has been processed. For requests within the 48-hour window, please contact our office directly at 435-776-6518. An AI assistant will answer, state you are calling for an urgent rescheduling or cancellation and it will transfer the call to our office staff (if within business hours).
